Welcome!
Welcome to my website. Office hours are by appointment so please call and I can work with you on providing a convenient time and date for your sessions. You can fill out the initial office visit paperwork and securely transmit it to me prior to your first visit. Simply click on the link below, fill out the entire form and click submit. If we have not scheduled prior to your form submission, I will call you to do so. If you prefer to fill out paperwork in the office, or you have questions about it, an additional 15 minutes is scheduled for all new clients before we begin the first session. The initial session, including completion of paperwork, runs 67 minutes. Proceeding sessions are 52 minutes unless your plan only reimburses for 45 minute sessions and are typically scheduled once weekly; however, multiple weekly sessions may be scheduled if we (you and I) believe it necessary. Insurance will cover these additional sessions in most circumstances.
**(This packet is not for the parenting classes. Click on the menu choice "Register For Parenting Classes" above to register for your court referred parenting class.)**
